CIRLS H-1B Online Questionnaire

Please click on the link below for the The Committee of Interns and Residents Legal Services (CIRLS) H-1B Online Questionnaire. The questionnaire should be filled in, printed out and then either:

1. Faxed to CIRLS at (212) 504-3057,

2. Scanned and e-mailed to CIRLS at proldan@cirseiu.org, or

3. Mailed to CIRLS at 520 Eighth Avenue, Suite 1200, New York, NY 10018.

Please also keep a copy of the questionnaire for your records.

Once you have supplied CIRLS with your filled in questionnaire and supporting documentation and checks (as detailed on the prior webpage under the heading "H-1B - Required Documents and Checks"), contact CIRLS at (212) 356-8195 and ask to speak to the Legal Department Secretary to schedule an H-1B interview with a CIRLS attorney. Please note that work will not begin on your H-1B case until CIRLS receives your H-1B Online Questionnaire and supporting documentation and a CIRLS attorney has interviewed you.
