• Gather all the relevant materials (such as receipts or explanations of benefit) and the appropriate claim form.
• Staple everything together with the claim form on top.
• Mail your claim directly to the Benefits Office.
• Do not submit or mail your claim to a representative of the hospital, as he or she is not involved in the processing of claims.

• The HSBP benefits for which claims may be made include: Hearing Aid, Mental Health Outpatient Benefit, Newborn, Optical, Prescription Drug, Smoking Cessation, Short Term Disability, Supplemental Dental, and Supplemental Obstetrical. The claim forms are available on the HSBP webpage.
• The PEP (Professional Educational Plan) claim form is available on the PEP webpage.
• The Book Reimbursement Benefit for WMC Residents and the Conference Reimbursement Benefit for WMC Residents claim form claim forms are available on the HSBP webpage.

• If you are employed at an HHC Hospital, you must use the PEP (Professional Educational Plan) claim form which is available on the PEP webpage.
• If you are employed at Westchester Medical Center, you must use the Book Reimbursement Benefit for WMC Residents claim form which is available on the WMC webpage.

• If you are employed at an HHC Hospital, you must use the Conference Reimbursement Benefit for HHC Residents claim form which is available on the HSBP webpage.
