Q : How do I submit a claim?
- Gather all the relevant materials (such as receipts or explanations of benefit) and the appropriate claim form.
- Staple everything together with the claim form on top.
- Mail your claim directly to the Benefits Office.
- Do not submit or mail your claim to a representative of the hospital, as he or she is not involved in the processing of claims.
Q : Where can I find claim forms?
A : There are 3 different kinds of claim forms, HSBP, PEP, and WMC:
- The HSBP benefits for which claims may be made include: Hearing Aid, Mental Health Outpatient Benefit, Newborn, Optical, Prescription Drug, Smoking Cessation, Short Term Disability, Supplemental Dental, and Supplemental Obstetrical. The claim forms are available on the HSBP webpage.
- The PEP (Professional Educational Plan) claim form is available on the PEP webpage.
- The Book Reimbursement Benefit for WMC Residents and the Conference Reimbursement Benefit for WMC Residents claim form claim forms are available on the HSBP webpage.
Q : Which is the correct form for a book claim?
A : There are 2 different kinds of book claims, PEP and WMC:
- If you are employed at an HHC Hospital, you must use the PEP (Professional Educational Plan) claim form which is available on the PEP webpage.
- If you are employed at Westchester Medical Center, you must use the Book Reimbursement Benefit for WMC Residents claim form which is available on the WMC webpage.
